Latest news says it was a maintenance guy who apparently forgot to reconnect 
together a small fuel line after working on the systems prior to the launch, 
which then leaked prior to the liftoff, causing the fire. Looks like there are 
no inherent design flaws in the rocket.

The full onboard video is now on the spacex site, all the way until it hits the 
ocean.
