There are several sources of incompleteness.

"Only the theory decides what one can observe.[1]"
is perhaps the main source.

But sometimes the theory cannot even decide
what one can observe, and one needs a superselection
rule, or a new principle, or whatever.[2].
Not to mention Anthropic Principles, etc.

[1] in W. Heisenberg, 'Der Teil und das Ganze',
R. Piper & Co., Munich (1969).
[2] see 'Pauli principle', which seems to be
physical (but, at the same time, the theory
says that wavefunctions are *not* physical
entities).
