To add some statistical references to what I was talking about with  
regard to US survival rates:

UK cancer survival rates are among the lowest in Europe, and all of  
Europe has significantly lower cancer survival rates than the US.  As  
a general pattern, a "life threatening" illness is significantly less  
life threatening in the US than elsewhere.  There are plenty of other  
problems in the US, mostly revolving around poor diet/lifestyle and  
high accident rate (due largely to excessive driving), but the  
quality of the medical care the average person receives is not one of  
those problems and actually compensates for a mediocre baseline.
