Higher and more challenging.
You start with a tutorial, then you continue with increasing difficult 
procedures.
Ten years; the input devices would be good enough to give a realistic 
force feedback and the models of internal anatomy much more developed 
and detailed.
Alone, a player could play any role in a surgery room (surgeon, 
anesthetist, ...) or they could setup a MultiPlayer where people could 
simulate a surgery room.
