I have proposed systems affording NOTA before. The most workable
solution I've found in this case is: the office is vacant upon the end
of the prior President's term, so the line of succession is followed
normally. There is no President or Vice President, but the new
Congress has already started so they have chosen a Speaker of the
House. Whoever that is, becomes President. Since NOTA's victory will
already be known at that point, in essence the House of
Representatives elects a President. (Whether or not NOTA is an option
in their election is likely a moot point: it seems like quite a
stretch to reach a situation where NOTA would gain more than a few
votes in their election; anyone in the House who would vote NOTA seems
far more likely to simply abstain.)
I'm not as certain about this next step, but I think that in this
case, the newly promoted one (after being sworn in) should (but is not
required to) select and appoint a Vice President.
