What is the reasoning followed by the designers of these spam filters?

Do they really think that a spammer who sends out millions of spam
emails is going to helpfully label all his messages with 'spam' in the
subject line?

In fact, have they ever, *ever* seen a real spam email with 'spam' in
the subject line?
