The H+ site just says it was created by an international group of authors.

There is a thesis pdf which quotes quite a long list of contributors,
but the link it gives goes to the H+ page which doesn't mention
individuals.

Does a thesis count as a reliable source?

Nick Bostrom's history says that the document was intended as a
consensus view of transhumanism, so maybe there were many smaller
contributors as well.
