The criteria varies, but generally requires a psych evaluation, a very
strong medical reason (alzheimer's, pain, etc.), and the assistance of a
doctor.  If I were going to die from an incurable disease in a very short
period (e.g., weeks), or was looking at massive brain damage from a
disease, then I'd go this route with cryonics in mind.
