Frankly, I don't think psychiatry is much more of a science than economics, sociology, political 'science', etc. Societal and professional biases are too prevalent.

Psychosis is not culturally defined but a physical disease affecting
the brain, as much as a stroke or a brain tumour is. As with most
things in medicine, there are cases where the diagnosis is obvious and
other cases where there is doubt. It is also not always clear to what
extent the psychosis contributed to the crime: I have known
schizophrenic patients who tell me that they steal things with
impunity because they are likely to be let off on account of their
mental illness.

Another consideration is that in most jurisdictions being found not
guilty for a serious crime such as murder on the grounds of insanity
is worse than being found guilty, in that it leads to longer sentences
in forensic prisons and more onerous monitoring and other conditions
once released.
