This appears to be a very comprehensive and cogent analysis of the

The overall message seems to be that (a) multiple backup systems did 
fail, but that (b) the in-depth planning was sufficiently deep that even 
with all of those backups failing, there were still enough lines of 
defence that the system is under control.

Give or take a few explosions.
