Well, an easy to solve many of these problems would be to just allow unrestricted immigration to and from the First World. This would allow people to quickly move from very bad locations to better ones.
This would be very unpopular and the welfare states would likely have to go, along with many regulations that restrict the flow of people and goods. All of this is very unpopular because most people in all societies have a deep seated antipathy toward foreigners. Bryan Caplan, in his _The Myth of the Rational Voters_, call this xenophobia "anti-foreign bias."
